For the first time in a report, the cause of the fire is mentioned.  It says the “suspected” cause of the fire is careless disposal of smoking materials.

Much like the NIOSH draft, Phase II of the Post Incident Assessment and Enhancement Team’s report points to incident command and building hazards for the deaths of nine firefigthters.

I talked with a source who attended one of the briefings this morning, and they said that Mayor Joe Riley and Chief Rusty Thomas attended the start of the meeting, and then left allowing the members of the Assessment Team to answer questions.
